How to request a copy of your transcript/diploma:

Current Students 
If you need a print out of your transcript, simply request it from the front office.
 
Post High School Aged (Graduated or Not)
If you need a copy of your High School Transcript or Diploma, you can:
  • Request directly from the front office, M-F, 8:00am-4:00pm
  • Request via Fax: (530) 822-5770, Attn: Records
  • Request via U.S. Mail: Albert Powell High School, Attn: Records, 1875 Clark Avenue, Yuba City, CA 95991
  • Request from either staff member below via email
 
The Following Information MUST be Included in the Request:
  • Date of Birth
  • Name while attending Albert Powell High School
  • Grad Year (or year in which you would have graduated
  • You may pick it up from the office or send someone else to pick it up on your behalf. If you elect someone else to pick up your copy of your transcript/diploma on your behalf, please include their name in the request. APHS will not give out personal documents to someone else unless authorized by the requestor.
  • If you would like your transcript sent via email or mail, please include the appropriate email address or mailing address in which you would like it sent.
